On Sat 24/5, we had an inspiring and heartfelt workshop with Nepalese community members, focusing on gambling harm and how we can support those affected. We are encouraged to actively raise awareness and support a wide range of communities, including Arabic, Portuguese, Brazilian and Vietnamese. We will inform you in the coming weeks about future events.

IIn 2023-2024, CCC provided 31,668 hours of services and 3,066 transport trips to elderly people, women, and families from culturally and linguistically diverse (CALD) communities, almost all of whom were financially and socially disadvantaged.

Who we engaged with
In 2023-2024, we engaged with 1,333 people and 52 organisational partners.

799 general community members
CCC placed focus on recovery and reconnecting the community and supporting
those most at risk of isolation and loneliness. We understand firsthand the importance of social and cultural connection for the wellbeing of individuals and communities.

170 CALD seniors over 65 years
The CCCi CHSP Services delivery is continuing consistently with our high standards of care, our services are compliant with all of Aged Care Quality and Safety Standards.

52 organisational partners
CCC’s work would not be possible without the financial support of these organisations: Inner West Council, Canterbury Bankstown Council, Addison Road Community Centre, NSW Government, Australian Government Department of Health, Australian Government Department of Social Services.

Our Hoa Mai Vietnamese Cultural Group continues to shine, sharing the beauty of Vietnamese culture through music and dance.

20 parents and children
The Inner West Vietnamese Family Group was a collaboration between CCC and Connect Marrickville (SaCC), where parents and children came together for joyful sessions.
IIt was heartwarming to see parents and children working side by side, creating beautiful memories and learning from one another. These moments of connection are what make our community so special.

Commonwealth Home Social Support Program
The Commonwealth Home Support Program (CHSP) at Community and Cultural Connections Ltd provides practical support to assist older people live independently and stay connected to their community. Through CHSP, we offer a range of services such as social support (individual and group), social outings, transport, and wellness activities that promote physical and mental wellbeing. Our team works closely with individual clients to design personalised services based on individual strengths, goals, and cultural and language needs. With a focus on dignity, choice, and community inclusion, our CHSP services aim to enhance quality of life and support healthy ageing.
Social Support Group Service (CHSP)
Community and Cultural Connections Ltd provides support for older people through the Commonwealth Home Support Programme (CHSP). Our social support services focus on each person’s strengths, needs, and goals to help them stay independent and involved in the community.
We work with clients, families, and the community to plan and deliver personalised, high-quality support. Our aim is to help older people stay physically and mentally well and live active, independent lives.
Our Group Social Support – Wellness Program includes:
Group activities at our centre
Social outings
We listen to what each client enjoys, needs, and prefers. We then work with trained facilitators to offer fun, meaningful activities and useful information.
Activities are based on what clients like to do and what they want to learn.
We also invite a range of guest speakers and presenters throughout the year—there’s always something for everyone!
How Much Does It Cost?
There is a small client contribution fee for this service. The fee is set in line with the CHSP Client Contribution Framework and may change over time. Please contact us for the most up-to-date information.

Social Support – Individual Service (CHSP)
This service helps older people who are frail stay connected with others and feel part of the community. It gives support and friendly company to people who may feel lonely.
We work with you, your family, or carer to make a plan that suits your needs. Our friendly staff and trained volunteers speak your language and can help with:
Phone calls or online check-ins
We can call you for a friendly chat, check how you’re doing, or remind you about appointments.
Home visits or going out together
We can visit you at home for a chat or go out for coffee. During visits, we can help you plan fun and healthy activities you enjoy.
Going with you to appointments or shopping
We can go with you to medical or social appointments, shopping, or to pay bills.
How much does it cost?
There is a small fee for each visit or accompany. This fee follows government rules under the CHSP Client Contribution Framework. Please contact us to check the current cost, as it can change from time to time.

Transport Service for Seniors
Community and Cultural Connections Ltd (CCC) offers transport services to help older people stay connected with their communities.
We support clients get to attend:
Centre-based day care (Social Support-Group)
Social outings
Medical and social appointments
We offer door-to-door transport (pick-up and drop-off from home) for people who receive Social Support – Group or Social Support – Individual services from CCC and live live within the Sydney Inner West Local Aged Care Planning Region.
A small fee (client contribution) applies.
How Much Does It Cost?
There is a small fee for each trip. The cost follows the Commonwealth Home Support Program (CHSP) Client Contribution Framework.
As fees may change from time to time, please contact our office for the most up-to-date information.
Transport includes individual pick-up and drop-off from home, subject to availability and capacity. Fees vary depending on the distance.

Kaissar
Looking back, the way Kaissar sees life is that “you get some good, some not so good, some bad, some not so bad – you can’t live all your life with only the good parts”. It’s the same perspective he applies to countries. Australia may not be immune from its own issues, but according to Kaissar it’s the ‘best country in the world’, which is why he has made it his home for more than five decades. .
Community Projects
Family support
in collaboration with Connect Marrickville School As Community, we maintained the support to Inner West Vietnamese families – 1,480 service hours face-to-face provided to 47 families with 91 people (parents and under five year-old children).

Women’s support
in collaboration with Leichardt Women’s Community Health Centre and Heartdancers organisation, we have provided emotional support and mental health wellbeing to vulnerable CALD women – 2,446 service hours delivered to 50 women.

Supporting Newly Arrived Migrants in the Inner West LGA
Assisted 84 newly arrived Nepali- and Vietnamese-speaking migrants by providing 6,552 hours of services to reduce social isolation, foster connections, and support their smooth settlement.

Community education and engagement
with CALD communities – 1,726 hours engaged with 1,397 community members.

Gambling Education and Awareness Project
Funded by the NSW Government through the Community Benefit Payment Scheme
In 2023-2024, we collaborated with 15 partners to implement gambling harm awareness initiatives for three multicultural communities—Nepali, Portuguese, and Vietnamese speakers. During this reporting period, the team conducted 27 community workshops, forums and engagement sessions, engaging 738 participants. Additionally, 665 Gambling Harm Help-Seeking Resource cards were distributed to interested individuals.

You’ve brought a sense of independence back into my life. These moments are more than just errands; they are a vital part of my wellness and reablement journey.
I love spending time with my CCC care worker, who teaches me and plays a variety of fun and challenging games with me every week. These activities help me regain my memories and keep my brain healthy. Thank you so much for your support care in this journey.
